Licensure application requirements by state

    • A completed and notarized licensure application form.

    • A personal check, or Cashier’s Check, or Money Order made payable to the Alabama Board of Examiners in Psychology in the amount of Four Hundred Dollars ($400.00) as a fee for processing the application.

    • A recent passport-quality photograph.

    • Official transcripts from the applicant’s entire education and training (beginning with Bachelor’s degree).

    • Appropriate references. The application requires four (4) positive references. Names of PSYCHOLOGISTS ONLY should be listed in the application; at least three (3) of the four (4) individuals providing a reference must be licensed or certified psychologists who also were licensed or certified at the doctoral level at the time of their interaction with the applicant. One (1) reference should be from the applicant’s dissertation advisor, departmental chair, or program director of the doctoral degree granting department, and one (1) should be from the pre-doctoral internship supervisor or director of internship training. If two (2) internship sites were used to fulfill minimum requirements, references must be received from an internship supervisor or internship director at each site. For applicants with prior professional experience, a reference from a supervisor or licensed colleague is required. When it is impossible to obtain a required reference, the Board may consider alternatives on a case-by-case basis. Current members of the Board are ineligible to act as references for applicants.

    • Applicants are required to have the Degree Validation Form (enclosed) completed by the Registrar, or graduate school director, or other approved official of the institution that granted the doctoral degree. The completed form should be sent directly to the Board from the institution.

    • Applicants that currently hold or have, in the past, held a license to practice psychology in another state/jurisdiction should send the Verification of Licensure Form to each licensing jurisdiction. The completed form should be sent directly to the Board from the other jurisdiction.

    • Applicants are required to complete and submit an Internship Documentation Form. This form is to be signed by both the Applicant and the Director of Internship Training. A maximum of two (2) internship sites or consortium programs may be utilized to meet the internship requirement. In instances where two (2) sites are used, a separate Internship Documentation Form should be submitted from each site.

    • Official transcripts sent directly from all undergraduate and graduate schools attended.

    • Verification of Licensure must be sent directly to the department from each jurisdiction where the applicant holds or has ever held a license to practice psychology. Each verification must include an explanation of any disciplinary action taken against the licensee. Check with each of the state boards you are or have been licensed in for their verification request process. If a state offers primary source verification on their website, that can be accepted as long as the website clearly confirms that it’s a primary source verification.

    • Reference letter(s) from immediate supervisor if a licensed psychologist, or two licensed psychologists who hold doctoral degrees. (#08-4362c)

    • A completed Master Education Course Work Check Sheet (#08-4362d).

    • A completed Psychological Associate Supervised Practice Plan (#08-4362e). Must be approved by the board before beginning supervision.

    • Complete from the date of high school graduation to the time of application, including dates and places of residency.

    • Upon completion of supervision, the Statement of Supervised Experience form (#08-4362f) must be submitted.

    • Proof of practicum in accordance with 12 AAC 60.084(2)

    • Applicant must submit Completed online application and non-refundable $350 application fee, paid online with submission of application. 

    • Completed and signed Arizona Statement of Citizenship and Alien Status form (SOC) (All applicants must sign form on page 2, Section IV.)

    • Current Government Issued ID. Not all IDs are the same. See the Evidence List (page 3 of the SOC form) for accepted forms of ID.  

    • Name Change Document (if applicable):  If any of your verifications such as transcripts, supervised professional experience (SPE) verifications, certifications, license verifications, etc. have a different name than the one on your ID, the legal document for the name change must also be submitted (e.g., court document, marriage license, etc.).

    • Fingerprint Clearance Card:  Statutes requiring a Department of Public Safety (DPS) fingerprint clearance card were enacted by the Arizona State Legislature in 2021.

    • National Practitioner Data Bank Self-Query - Please provide a PDF copy of a new self-query from the National Practitioner Data Bank (NPDB). The processing date in the upper right corner of the self-query must within 30 days of your application submission date in order to be valid.  Upload the original PDF you download from the NPDB's website. Do not upload a copy or an image - the official document with digital signature from the NPDB is required.

    • Photograph: One original “passport quality” photograph taken not more than 60 days before the date of your application submission.

    • Official transcripts from all graduate educational institutions attended, regardless of degree, sent directly to the Board by the university/college.

    • Internship Verification:  Verification of minimum 1,500 hours of Internship, which was completed within 24 consecutive months and meeting all other requirements for internships in A.R.S. Section 32-2071

    • Preinternship Verification:  Verification of Preinternship, if you are applying preinternship hours towards licensure (A.R.S. 32-2071(D)). Verifications must be submitted by the director of clinical training or similar official in order to be valid.  Applicants must complete Section A and B of the preinternship form. The director of clinical training must verify Section B in Section C for each preinternship site, and then complete Sections D and E

    • Postdoctoral Verification:  Verification of Postdoctoral Supervised Experience, if you are applying postdoctoral hours towards licensure. Applicants must complete the top portion of the Postdoctoral Verification form and submit it to their supervisor or current licensed psychologist who is the official custodian of records for the postdoc. 

    • License Verifications: If applicable, verification of all psychology licenses and other professional licenses (may also be referred to as registrations or certifications) ever held in any states/provinces, sent directly to the Board of Psychologist Examiners by the issuing agency or board. This includes PSYPACT. All licenses ever held must be verified regardless of current status

  • STEP 1

    • Complete a 2000-hour internship that meets the criteria specified in Section 5.4.F. of the Rules.

    • Download an Application Request Form. Complete this fillable form, then email it to the office at APB.Info@arkansas.gov. You will be assigned an applicant number.

    • To receive an application the fee is $200. You can pay it all or submit an initial $50 deposit for the packet, then make a final $150 payment upon submission of the full application.

    • After receiving an applicant number you can pay online:  PAY NOW

    • Submit your application to the board office. Also include two checks for your background check fees made payable to Arkansas State Police.

    STEP 2

    • If the core courses are approved, an approval letter from the board will be emailed to you granting you permission to schedule the national exam through the exam provider. The administrative cost for the registration to take the EPPP is $50, payable online. Scheduling the date and time of the exam is the responsibility of the candidate for licensure, not the board.

    • The examination is administered by an exam service via computer.

    • Scores will be reported to the applicant upon completion and to the Board office directly from the exam service.

    • Passing score is a minimum of 500. If candidates do not pass the exam, they must wait a period of 60 days before taking exam again. The EPPP can be taken a maximum of 4 times over a period of one year.

    • The statement of intent, also referred to as the scope of practice, is in your application packet, or you can download it. If you want to revise it at any time in the future, you must submit an updated form including ALL areas of practice, not just the updates.

    • Three (3) Reference Forms and One (1) Letter from Program Director. One letter from the Director of the program at the institution where your academic work was obtained.  It should document successful program completion.

    • Three recommendations must come from qualified Psychologists with direct and comprehensive knowledge of your training and competence. Family and close friends are excluded.

    • Fingerprint Card Process. The board office will email fingerprint packets to in-state individuals. All in-state individuals will need to go to Arkansas LiveScan to have their fingerprints and backgrounds done. If you are out of state individual the board office will send you a fingerprint card and packet to complete.

      Please only mail the fingerprint card and packet back to the board office if you are an out-of-state individual

    • Official University Transcript(s)Order the official university transcript(s) of doctoral-level coursework to be sent to the board office. The official transcript conferring the completion of the degree must be on file with the board prior to scheduling an oral exam.

    • Out of State Licensees. A license verification form must be completed. Please complete the top portion of the form and send to either the state board that issued your license to include the issuance and the expiration date.

    • If you have taken the EPPP please have ASPPB send your score to the Arkansas board.

    • Obtain a qualifying doctoral degree

    • Accumulate a total of 3,000 hours of SPE, with at least 1,500 hours post‑doctorate

    • Post‑doctoral SPE must be accrued within 30 months after meeting all doctoral requirements, confirmed by your registrar or training director

    • Submit an Application for Licensure as a Psychologist via BreEZe (online) or by mail, along with:

      • Application fee

      • Official transcripts

      • Verification of at least 1,500 hours of SPE (pre‑ and/or post‑doctoral)

      • Supervision Agreement & VOE (sealed)

    • Submit a CPLEE Request Form (online via BreEZe or by mail) with associated fee.

    • Provide verification of the full 3,000 SPE hours (including at least 1,500 post‑doc), with Supervision Agreement & VOE

    • Submit fingerprints for a criminal background check via Live Scan (if in California) or “hard card” method (if out of state) to both DOJ and FBI—this is required prior to licensure

    • After passing both EPPP and CPLEE, you will receive the Request for Initial Licensure form.

    • Submit the completed form with the required fee to the Board

    • Doctoral Degree in Psychology - APA or equivalent

    • Apply for a Psychologist Candidate (PSYC) Permit. Enables you to legally accrue supervised postdoctoral experience and take the required exams.

      Application includes: transcripts, a $40 fee, and—for degrees not yet conferred—a Certification of APA Doctorate Education form

    • Complete 1,500 Hours of Post-Doctoral Experience. Must span at least 12 months. Up to 500 hours teaching, and/or 500 hours research (both under supervision)

    • At least 75 hours of supervision required, with 50 hours being face-to-face, individual supervision

    • At least 50 hours must address racial/ethnic bases of behavior, with at least 3 hours of supervision in that area (or coursework may be permitted)

    • Supervisors must complete a Post-Degree Experience and Supervision Form, which will be submitted with your full licensure application

    • Take EPPP - 500 or higher

    • Take Colorado Mental Health Jurisprudence Exam: Aa45-item, multiple-choice, open-book online exam focused on Colorado-specific mental health law. Passing scaled score is 500 or higher.

    • Submit Application for Full License. Submit via the online portal with required documentation and a $70 application fee

    • Obtain Doctoral Degree in Psychology. Must be from an APA-accredited program during the time you attended; if not accredited, your program will undergo individual review per Section 20‑188‑2 of the Connecticut Regulations.

    • Minimum of 1 Year of Supervised Work Experience. Must be pre- or post-doctoral (internship hours required for degree work don’t count). At least 35 hours/week for a minimum of 46 weeks (12 consecutive months), or at least 1,800 hours over 24 consecutive months

    • For every 40 hours of experience, at least 3 hours of supervision—including at least 1 hour of individual, face-to-face—is required.

    • Instead of the supervised experience, applicants may submit documentation of 2 years of licensed work experience for eligibility.

    • Submit Application to DPH. Includes:

      • Notarized application form (with passport-style photo).

        Official transcripts (sent directly from your institution).

        Verification of supervised experience or documentation of 2 years licensed work (if substituting).

        Applicable fees (approx. $565).

    • Register for the EPPP (typically $600), and upon passing, you may proceed with the jurisprudence exam.
